In patients with PSPA-nt, the presence of specific, suggestive, and nonspecific symptoms of TD were assessed again at the time of prolactin normalization. In addition, we excluded patients with mildly elevated prolactin levels attributed to a pituitary stalk compression and those with untreated anterior pituitary deficiencies. To take into account the biologic variability of testosterone assay and the clinical significance of this marker's evolution, we decided to include an age-, BMI-, and follow-up duration-matched cohort of individuals harboring a nonprolactin-secreting pituitary adenoma (PA-nt). From a large cohort of patients harboring PSPA, we included those having, on 2 or more occasions, serum total testosterone concentrations ≥3.0 ng/mL (≥10.4 nmol/L, PSPA-nt), according to generally recommended clinical practices 11, 12.
CNPA is generally considered a clinical subtype of ischemic pituitary apoplexy with unique intraoperative cosmetic appearance as well as histopathological features30,31. Our study showed that CNPA is one of the unfavorable factors for HPG axis recovery in patients after surgery. Therefore, early surgery and shortening pituitary ischemia time are of positive significance for testosterone recovery.. The pituitary gland normally produces GH, but a tumor on their pituitary can produce excess GH in gigantism.|When pituitary gland disorders occur, they can have a significant impact on testosterone levels and overall men’s health. Circulating prolactin levels at baseline were not different between patients improving and in those not improving testosterone levels, indicating the absence of a threshold effect and arguing for an inter-individual intrinsic resilience of the HPG axis. In addition, contrary to PA-nt patients, the proportion of PSPA-nt experiencing specific and suggestive symptoms of TD patients significantly decreased after a successful treatment aiming at normalizing prolactin.|Some men with other types of pituitary adenomas, such as nonfunctioning pituitary adenomas (NFPAs), tend to have low gonadal hormone test results even in the absence of elevated PRL. Female patients can present early due to elevated PRL, causing irregular menstruation or lactation. HPG axis involvement may be caused by hyperprolactinemia and tumor compression. Patients often present with clinical manifestations arising from endocrine and mass effects depending on the tumor type2,3. Tumor size, CNPA and invasiveness were predictors of testosterone recovery in postoperative patients. It plays a crucial role in regulating various hormones in the body, including testosterone.}
Instead, it helps to maintain normal bone, cartilage and organ structure and metabolism. Growth hormone, also known as human growth hormone (hGH) and somatotropin, is a natural hormone that acts on many parts of the body to promote growth in children. However, the optimal amount of testosterone is far from clear. As a result, there is some controversy about which men should be treated with supplemental testosterone.
Pseudoacromegaly is a condition with the usual acromegaloid features but without an increase in growth hormone and IGF-1. An MRI of the brain focusing on the sella turcica after gadolinium administration allows for clear delineation of the pituitary and the hypothalamus and the location of the tumor.citation needed After diagnosis, medical imaging of the pituitary is carried out to determine if an adenoma is present. have been undertaken on the relationship between more general aggressive behavior, and feelings, and testosterone. Testosterone levels play a major role in risk-taking during financial decisions.|Seventy-one (55.9%) patients recovered testosterone levels without developing new hypopituitarism after surgical resection of the tumor via pseudocapsule. According to the postoperative testosterone classification criteria, 127 of 291 (43.6%) patients had low preoperative testosterone, in which testosterone levels returned to the normal range at 3-month postoperative follow-up in 71 (55.9%). The diagnostic criteria for patients with central hypogonadism are low testosterone levels, accompanied by inappropriate normal or low gonadotropin levels. In clinical practice, as an indicator for efficacy determination, changes in prolactin (PRL), cortisol, growth hormone, and thyroid hormone often gain more attention, and testosterone is relatively less assessed. Seventy-one of 127 patients with low preoperative testosterone levels recovered to normal levels. In conclusion, pituitary gland disorders can have a significant impact on testosterone levels and overall men’s health. The pituitary gland acts as the master gland, controlling the production and release of various hormones, including testosterone.|If your child’s healthcare provider thinks they may have gigantism, they’ll likely recommend you see an endocrinologist, a healthcare provider who specializes in hormone-related conditions. Gigantism can also be caused by pituitary hyperplasia, which is when your pituitary gland becomes enlarged. Growth hormone (GH) regulates growth during childhood by promoting measured bone, muscle and tissue growth and helping to control metabolism. Though it’s very rare, gigantism can affect any child whose growth plates in their bones haven’t fused yet (they haven’t finished puberty). In children, gigantism occurs when they experience excess GH before the growth plates in their bones fuse (before the end of puberty). Gigantism and acromegaly are both conditions that result from excess growth hormone (GH).}
Apart from the elevated gonadotrophins and mild elevation of prolactin due to deviation of pituitary stalk, other pituitary hormones on presentation were normal except of mild central hypothyroidism in one case . In addition, data on hormone tests did not contain information on sex hormone binding globulin and serum free testosterone results, which may be more beneficial if further assessment of sexual function in male patients is required. Therefore, we hypothesize that the application of pseudocapsule technique may have a significant promoting effect on the recovery of gonadal hormones in male patients after surgery.
